ABSTRACT OF JUDGMENT - A judgment can be recorded as a lien against a property. This form is submitted to the court, and once filed, a certified copy is filed with the Clark County Recorder's Office.
ACCEPTANCE OF SERVICE OF AFFIDAVIT OF COMPLAINT - A form signed by the defendant acknowledging that they have been served a copy of the complaint.
AFFIDAVIT OF RENEWAL OF JUDGMENT - A judgment can expire. A request to renew the judgment must be filed at least ninety (90) days before the judgment's expiration date.

AMENDED COMPLAINT - Filed when a plaintiff changes the information in their original complaint. The amended complaint must be filed and served on the defendant(s).

AMENDED COUNTERCLAIM - Filed when a party changes the information in their counterclaim. The amended counterclaim must be filed and served on the other parties.

APPEAL OF SMALL CLAIMS JUDGMENT - Filed to request that the case be reviewed by the District Court. A $250.00 bond or the entire amount of the judgment against you may need to be posted with the court.

APPLICATION TO RESET EXPIRED COURT DATE - A request to reschedule a hearing that has already passed.
CHANGE OF ADDRESS - Filed to change a party's contact information.

CLAIM OF EXEMPTION FROM EXECUTION - If the winning party attempts to collect the judgment, the losing party can request that certain property be exempted from seizure under Nevada law. This must be filed within ten (10) days of when the notice of attachment or garnishment was mailed.
COMPLAINT - The complaint opens the case. It must be served on the defendant after a trial date has been entered on the summons at the bottom of the complaint. A copy of the demand letter or some request for payment should be filed along with the complaint.

COUNTERCLAIM - Filed if a defendant wishes to sue the plaintiff AFTER the plaintiff has sued them.

MOTION FOR ORDER ALLOWING EXAMINATION OF JUDGMENT DEBTOR - Filed after a case is over, this motion compels the losing party to appear in court and give the winning party information about their property, accounts, and earnings.

MOTION TO PLACE ON CALENDAR - Filed when something in the case needs the attention of the judge. This form is asking the judge to set your request for hearing.

MOTION TO RELEASE EXECUTION - The winning party can request that money held under a writ be released. If it is granted, the court will issue an order releasing the funds.

MOTION TO SET ASIDE DEFAULT JUDGMENT - Filed after a default judgment has been entered, this asks the court to cancel the default judgment and/or set the case for a hearing.

MOTION TO SET ASIDE DISMISSAL - Filed when the case was dismissed, this motion asks the court to reconsider. The court can set this for hearing or decide on it without a hearing.

NOTICE OF BANKRUPTCY ACTION - This form can be used if a defendant filed a bankruptcy and attaches either a Notice of Filing, a Notice of Dismissal, or a Notice of Discharge.

NOTICE OF VOLUNTARY DISMISSAL - Filed when the plaintiff does not wish to continue with the case. A dismissal "with prejudice" means you cannot file any other actions on this matter. A dismissal "without prejudice" means you can file a new claim for the same matter.

OBJECTION TO CLAIM OF EXEMPTION FROM EXECUTION - Filed by a judgment creditor (person who is owed money). It is a response to a Claim of Exemption from Execution filed by the judgment debtor (person who owes money). This form must be filed within 8 days of the service of the Claim of Exemption from Execution.

OPPOSITION (GENERIC) - Filed if you are opposing a motion that the other party filed. It must be filed and mailed to the other side.

PROOF OF SERVICE (FOR COUNTERPLAINTIFF)
Filed after the counterclaim is filed. The party that serves the counterdefendant with the counterclaim will fill this form out, and the counterplaintiff will file it with the court.

PROOF OF SERVICE (GENERAL) - Any party can use this form to show that a document was served.

PROOF OF SERVICE (FOR PLAINTIFF) - After the complaint is filed and a trial date has been entered on the summons at the bottom of the complaint, plaintiff should give this proof of service form to the constable, sheriff, process server, or other neutral third-party.

SATISFACTION OF JUDGMENT - Filed if a judgment has been paid and satisfied by the losing party.

SUBPOENA - If you are requesting the appearance of a witness or a witness to bring documents to your trial, fill out this form and bring it to the Justice Court clerk's office for issuing. Check with the court in which you are filing to find out if there are witness fees that must be included.

WRIT OF EXECUTION - This form must be typed. Once this form has been filled out and filed, the Notice of Execution After Judgment must be attached as pages 3-6.

WRIT OF GARNISHMENT
This form must be typed. It is not filed with the court, but after it is completed, it is given to the sheriff or constable along with the filed Writ of Execution.
